Last updated November 8, 2026
Duricor, LLC (“Duricor”, “we”, “us”) runs this patient portal and the Duricor Health Monitoring service. Patients of Dr. Jeffrey Chen’s practice use them to record their health readings and share them with their care team. This policy explains what information we collect, how we use and share it, and the choices you have. It covers:
- this website and patient portal (drjchen.com);
- the Duricor mobile app;
- our automated phone line for logging readings;
- the text messages and emails we send you.
We do not sell your personal information, and we do not use it for advertising.

1. Your health information
The readings, recordings and reports in this service are part of your medical care. We handle them on behalf of your care team and protect them as required by the Health Insurance Portability and Accountability Act (HIPAA). Your care team’s Notice of Privacy Practices explains your rights over your medical information. Where that notice and this policy differ, the notice applies to your medical information.
2. Information we collect
From you and your care team
- Your name, date of birth, sex, home address, phone numbers and email address.
- The PIN you use to sign in to the phone line, and your portal sign-in details.
Health information you record
- Readings such as blood pressure, blood sugar, heart rate, weight, water intake and steps.
- Symptoms, exercise and stress minutes, food logs and questionnaire answers.
- ECG (EKG) recordings that your heart monitor records and sends to your care team.
- The reports and alerts created from this information for you and your care team.
When you use the phone line or text messages
- The phone number you call or text from, the readings and answers you give, and the date and time.
When you use the mobile app
- The app uses your phone’s Bluetooth to connect to your heart monitor. To connect the monitor to the internet, the app asks for your Wi-Fi network name and password; we use them only to set up your monitor.
Automatically, when you use the website
- Our servers record your IP address, browser type, the pages you open and when, to keep the service working and secure.
We do not buy information about you from other companies.
3. How we use your information
- To provide the service: show your readings to you and your care team, prepare your reports, and alert your care team to readings, such as blood pressure, that need attention.
- To send you reminders, notices about the service, and replies by text message and email (text messages only if you agreed to receive them; see Text messages).
- To help you when you contact us.
- To keep the service secure, and to find and fix problems.
- To meet our legal obligations.
4. Who we share it with
- Your care team: the doctors, nurses and staff at your practice.
- Service providers that run parts of the service for us under contract and only on our instructions: Amazon Web Services (hosting, data storage, the phone line, text messages and email), and an error-monitoring service that receives technical details when something goes wrong. We remove patient details from those error reports before they are sent.
- When the law requires it, or when needed to protect someone’s health or safety.
- If Duricor is merged or sold, as part of that transaction, with the same protections.
We never sell your information or share it with anyone for their own marketing.
5. Text messages
If you agree to receive them, Duricor Health Monitoring sends text messages to your mobile phone: reminders to log your readings, replies when you text us your readings, and occasional notices about the service.
- Your choice. We only send these texts if you agreed to receive them. Agreeing is not required to receive care.
- How often. Message frequency varies. Most texts are replies to messages you send us.
- Cost. Message and data rates may apply, depending on your mobile plan.
- Stopping. Reply STOP to any message to stop receiving texts. Reply HELP for help, or contact us.
Your mobile number stays private. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-message opt-in data and consent will not be shared with any third parties, except the service providers that deliver our messages for us.
The full terms for our text messages are in our Terms of Use.
6. Emails
We may email you about your account and the service, for example when a new report is ready. If you sign up for news and health information by email, you can unsubscribe at any time using the link in the email or by contacting us.
7. Cookies and website technology
We use cookies only to keep you signed in and to protect our forms. We do not use advertising or tracking cookies, and we do not track you across other websites, so our site works the same whether or not your browser sends a “Do Not Track” signal.
Our pages load their fonts from Google Fonts, so Google receives your device’s IP address when a page loads.
8. How long we keep your information
We keep your information while you are enrolled in the service and afterwards for as long as medical record and other laws require. When we no longer need it, we delete it or remove the details that identify you.
9. How we protect your information
We use technical and organizational safeguards, such as encrypted connections and access limited to the people who need it. No system is completely secure, so we cannot guarantee that information will never be accessed without permission, but we work to prevent it and will tell you if the law requires.
10. Your choices and rights
- See your readings and reports at any time by signing in to the patient portal.
- Ask us or your care team for a copy of your information, or to correct or delete it. Some information must be kept in your medical record by law.
- Stop text messages by replying STOP, and stop news emails with the unsubscribe link.
Depending on the state you live in, you may have further rights, such as to know what we hold about you, to get a copy, to correct or delete it, and to appeal if we turn down your request. To use any of these rights, contact us. We will check your identity before acting on a request, and you may ask someone you authorize to make it for you. If we turn down your request, you can appeal by emailing us, and if your appeal is denied you can contact your state attorney general. We will not treat you differently for using your rights.
11. Children
This service is for adults. We do not knowingly collect information from children under 18. If you think we have, please contact us.
12. Changes to this policy
We will update this policy when our practices change and show the new date at the top. If a change is significant, we will also tell you on this website or by message.
